For ACA preventive services with your assigned PCP — no. For preventive screenings done by a specialist (e.g. dermatology), Navigate plans typically require a PCP referral first. Choice and Choice Plus generally do not.
Same federal ACA mandate applies. Oxford (NY/NJ/CT) and AllSavers (level-funded employer plans) both cover the standard preventive services at $0 in-network.
